When you first contact Liv Associates, we will ask for some insurance information so we can verify your eligibility and determine your deductible, co-payment, and co-insurance. Appointment Cancellation Policy

We understand that plans sometimes change. If you need to cancel or reschedule an appointment, please provide at least 24 hours notice.

To help you stay on track, we send two appointment reminders — one two days before your appointment and one the day before. These notifications are a courtesy to help you plan ahead and ensure you have plenty of time to reach out if anything changes.

Giving advance notice respects your therapist's reserved time and allows us to offer the appointment to another client who may be waiting for care.

We also understand that emergencies and unexpected circumstances happen. If something prevents you from attending, please contact us as soon as possible. Our team will be happy to discuss your situation with you.

Good Faith Estimate

Effective January 1, 2022, you have the right to receive a Good Faith Estimate for the expected cost of your care.

What is a Good Faith Estimate?

A Good Faith Estimate shows the expected costs of items and services for your health care needs based on information known at the time it is created. It does not include unknown or unexpected costs that may arise during treatment.

You have the right to receive a Good Faith Estimate before your scheduled service or at any time upon request.

If you are billed more than your Good Faith Estimate, you have the right to dispute the bill.

You may contact us directly to update the bill, negotiate the charges, or ask about financial assistance.

You can start a dispute with the U.S. Department of Health and Human Services (HHS) within 120 days of the date on your original bill. There is a $25 fee to use the dispute process.
